Book a Routine Appointment
All our physicians offer virtual, phone, or in person appointments.
Whether you have a chronic issue like diabetes, or a new health issue, you can book an appointment by clicking on the button below.
If you are having a medical emergency, we advise that you should go to the emergency room.
We are working to improve our access to appointments. Depending on the purpose of your booking, you will receive a ‘Qnaire’ through our portal. This will enable you to answer questions before your appointment which will help your physician better care for you.
Urgent Appointments
Our physicians offer same-day appointments that open up the day of.
If you are having a medical emergency, we advise that you should go to the emergency room.
Additionally, we offer an after hours clinic as well on Monday to Thursday, and Saturday Morning.

How we get information to you
Email notifications:
To make sure email notifications do not go to your “Junk” folder, please add the following emails to your contact or safe list: noreply@inputhealth.com & notifications@yourhealthvotresante.ca
Text notifications:
Certain notifications may be available via text message as well, please ensure your mobile number and email address are kept up to date on our system (you can update this yourself by logging into the portal, using the CHR Connect App, or by calling the office at 613-366-7557 and leaving a message.
Mass emails for Clinic events:
You will also get emails from us from time to time about clinic events (such as availability of the flu or COVID vaccines) or important public health announcements.
Are these services covered by OHIP?
Digital services, like messaging and online booking are not an insured OHIP service. However we currently do not charge a fee for them. We would like to keep this service free for everyone to be able access equally. However this may change depending on future economic circumstances and future increases
Online booking
We make the most of online tools and technology to enhance our services. We use our website and portal for booking appointments and sending secure messages, which helps us work more efficiently and provides easier access for you. That means we can spend more time on your care. We encourage patients to actively participate in their healthcare by completing pre-visit questionnaires (we call them ‘Qnaires’) that will be automatically sent to you before your appointment (if needed).
For certain results, we will send a secure notification from our patient portal. Please check your messages regularly to receive and review these messages in the Patient Portal.
Being on time
Time is valuable. We prioritize running on time. If patients are late, they may have to re-book and/or a charge will apply. Patients are advised to allow up to a 60 minute buffer due to the unpredictable nature of medicine.
We will remind you with automated email reminders before your appointment. You can change or cancel your appointment through our online portal.
48 business hours notice of appointment changes or cancellations is required or we may charge a Late Cancellation or Missed Appointment fee. If there is an emergency and you can not make your appointment contact us as soon as possible.
